• CLICK ON THE LINK BELOW AND PRINT THE FORMS.

• Read the index page throughly for instructions on how to complete each page. Make sure they are filled out completely and accurately.

• The packet MUST BE COMPLETE and you must be cleared by the Athletic Trainer and Athletic Department before you can practice or be put on an eligibility rosters.

• PHYSICAL'S ARE MANDATORY EACH YEAR, but MUST be completed after JULY 1st.

• Turning in an Athletic Participation Packet doesn't automatically deam you eligible to compete in intercollegiate athletics. You must meet all NWAACC or NJCAA eligibility requirements.

• Once complete, either mail them to your coach or if you'll be arriving on campus soon--bring them with you. (Pages 1-7 to the Athletic Dept. and Pages 8-16 to Athletic Trainer).
