Staff Tuition & Fee Benefit Request

Please complete a form for each term (do not combine terms). Supervisors need only sign IF the course being requested for is job-related. Please submit to HR for initial processing.
Dependent Tuition & Fee Benefit Request

This form must be submitted each term to entitle your dependent to take a class.
Part-time Faculty Tuition Benefit Request Form

Must be completed each term you take a class/classes. Instructions on accrual and usage of tuition credit is included in the form. For verification of available credits, please contact Human Resources.
Retiree Tuition Waiver Request
