Staff Tuition & Fee Benefit Request (65 KB)

Please complete a form for each term (do not combine terms). Supervisors need only sign IF the course being requested for is job-related. Please submit to HR for initial processing.

Dependent Tuition & Fee Benefit Request (45 KB)

This form must be submitted each term to entitle your dependent to take a class.

Part-time Faculty Tuition Benefit Request Form (51 KB)

Must be completed each term you take a class/classes. Instructions on accrual and usage of tuition credit is included in the form. For verification of available credits, please contact Human Resources.

Retiree Tuition Waiver Request (43 KB)

To be submitted each term a class/classes is taken.

Dependent Tuition Waiver Eligibility Form (41 KB)

To be completed ONCE to verify your eliglible dependents for tuition benefits. Update as necessary.
