ODS Forms

• Affidavit of Dependency - for children/dependents who are NOT your biological or adopted children of the insured.
• Affidavit of Domestic Partnership
• Mail-Order Prescription Form (Wellpartner) - for mailing in new prescriptions each time you need a refill.  To maximize your benefit, you might want to show your doctor OEBB's Preferred Drug List for possible cheaper alternatives.
  (NOTE: You may also register an account with Wellpartner via their secure website at www.wellpartner.com where,after registration is complete, your doctor may fax prescriptions to Wellpartner at 1-866-624-5797.)

• Enrollment/Change Form  - This form should only be used when instructed. You may add eligible dependents through the OEBB website at www.myoebb.org. If you need help with registering or recalling your userid & password, call us at ext 7280/7259/7204.
