Reference checks for potential hires need to be conducted before offering the position. These forms are created to keep your questions consistent so as to comply with EEO regulations. We require reference checks from three (3) former supervisors. If you have any questions, please contact Human Resources for clarification. Please keep these forms confidential and return them to Human Resources for archiving.
