Satisfactory Academic Progress Forms

• Probation status requires the following form to be completed and submitted to the Financial Aid Office.

• Aid Withheld status requires the following form to be completed and submitted to the Financial Aid Office. The request will be submitted to the Financial Aid Committee for review, and you will receive a decision notice from the Financial Aid Office. The review and decision usually occurs within one week after all forms and documentation have been received. Please allow additional time at the beginning of a term.
