First Time Borrower Loan Request Form

Complete the entrance counseling session.
After successfully completing the entrance, you will be linked back to Southwestern’s web page where you will need to choose your lender. This will link you to the lender’s web page where you will complete the master promissory note (MPN) online and electronically sign the note.

You must sign the master promissory note, which is a promise to repay the loan amount you receive. By signing this note, you also authorize Southwestern to credit loan funds to your student account.

Determining your income and expenses is essential when requesting a loan. To determine how much you need, list the income and expenses you will have for the terms you are requesting to borrow by completing the budget below.
